STANDING ORDERS:<br />
<strong>Navilyst</strong> <strong>Medical</strong> accepts standing orders on all products. This convenience provides you with scheduled shipments using a single purchase order. Contact the<br />

RETURNED GOODS:<br />
Permission to return merchandise is required for all returns or exchanges. All items must be in unopened packages and will be subject to a 20% restocking<br />
charge. Returns and exchanges will only be accepted within ninety days of the date of purchase. Material that has been opened, damaged, or tampered with is<br />
not returnable or exchangeable for credit. Products manufactured as SPECIAL orders are not returnable or exchangeable for credit. <strong>Navilyst</strong> <strong>Medical</strong> will not<br />
authorize returns for expired products or products with less than six months of shelf life remaining. All merchandise returned to <strong>Navilyst</strong> <strong>Medical</strong> must be<br />
accompanied by a copy of the packing slip or invoice enclosed with the original shipment. The Returned Goods Authorization (RGA) number must be clearly<br />
marked on the shipping container. Merchandise returned without an RGA number will not be accepted or considered returned.<br />

• Identify the customer account number, catalog number, size, quantity, lot number, invoice number and the reason for the return.<br />

or resterilization may also create a risk of contamination of the device and/or cause patient infection or cross-infection, including, but not limited to, the transmission<br />

assumes no liability with respect to instruments reused, reprocessed or resterilized and makes no warranties, express or implied, including, but not<br />
