Welcome to <strong>Contra</strong> <strong>Costa</strong> <strong>Health</strong> Plan<br />
This directory is a list of <strong>Contra</strong> <strong>Costa</strong> <strong>Health</strong> Plan’s medical services and where you can find them. You will<br />
find:<br />
Doctors and Family Nurse Practitioners you can choose listed by location AND in alphabetical order. If a<br />
doctor or family nurse practitioner speaks a language other than English, it will be in the listing. If staff at<br />
the doctor’s office speaks a language other than English it will be in the listing.<br />
Hospitals, pharmacy and specialty services listings<br />
This Provider Directory helps you find the right medical provider for you. As a member of the <strong>Health</strong> Plan<br />
you can choose your Primary Care Provider (PCP) from the Regional Medical Center Network or the<br />
Community Provider Network. To choose or change your PCP call your Member <strong>Services</strong><br />
Representative at 1-877-661-6230 (press 2).<br />
County Employees who have enrolled in Plan A can only select a Primary Care Provider from the Regional<br />
Medical Center Network. County Employees who have enrolled in Plan B can select a Primary Care Provider<br />
from the Regional Medical Center Network or the Community Provider Network. Other provider services will<br />
be listed for Plan A and/or Plan B access.<br />
What’s a PCP?<br />
You may be new to a health plan. You will hear words like Managed Care, Primary Care Provider, and<br />
Provider Network. Here are a few things you should know…<br />
MANAGED CARE means you will have your own personal doctor to oversee, coordinate and provider<br />
all your necessary medical care services. This will help you and your family get the right care at the right<br />
time, in the right setting.<br />
YOUR PRIMARY CARE PROVIDER (or PCP) is another name for the doctor that you will see most<br />
of the time when you need medical care. You may also receive your care from a nurse practitioner. If you<br />
need to see a specialist, your Primary Care Provider will arrange all authorized referrals and help pull it all<br />
together for you.<br />
IF YOU NEED HOSPITALIZATION you will probably go to the hospital where your PCP has<br />
admitting privileges. Your Primary Care Provider will decide when you need to go to a hospital and<br />
which hospital is best able to care for you.<br />
CHANGING PCPs is easy. All you need to do is call Member <strong>Services</strong> 1-877-661-6230 (press 2) and<br />
they will help you choose another Primary Care Provider. Generally, it is best not to change doctors while<br />
you are pregnant, hospitalized or undergoing a course of treatment.<br />

GO TO THE TABLE OF CONTENTS and look for the listing “Primary Care Providers by City.”<br />
SELECT A CITY and go to the page(s) for that selection.<br />
LOOK AT THE PROVIDERS LISTED – If the provider is under the Heading “Regional Medical<br />
Center Network” both Plan A and Plan B members can select that doctor. If the Provider is under the<br />
heading “Community Provider Network” you may only select that doctor if you are a Plan B member.<br />
CALL THE MEMBER SERVICES DEPARTMENT at 1-877-661-6230 (press 2) to confirm your<br />
selection.<br />
For CCHP PLANS: Plan A and Plan B
