A. HOW TO FILE AN STD CLAIM<br />

The STD claim process consists <strong>of</strong> four steps. It is your responsibility, as<br />

an employee, not your department’s, to initiate and timely file your STD<br />

claim. This is accomplished by contacting Sedgwick no later than the last<br />

day <strong>of</strong> your Waiting Period. Please refer to the definition <strong>of</strong> Waiting Period<br />

on page nine for more information. Your treating health care<br />

practitioner must submit the physician’s certification form, or call<br />

Sedgwick within 29 days <strong>of</strong> your disability start date or your claim<br />

will be considered filed late. Failure to complete these contacts<br />

timely will result in the loss <strong>of</strong> your STD benefits for each day your<br />

claim is late until both contacts are successfully completed. Please<br />

refer to the definition <strong>of</strong> Waiting Period on page nine for more information.<br />

Step 1: As soon as you become Disabled, or miss work due to an<br />

illness, call Sedgwick between 8:00 a.m. and 5:00 p.m.,<br />

Monday through Friday, at: 1-800-786-8600<br />

or contact them on the Internet, at:<br />

www.SedgwickCMS.com/Calabasas<br />

A Sedgwick representative will take the necessary information concerning<br />

your disability over the telephone. If you are too ill to call personally,<br />

another person may call for you. You or your representative will be asked<br />

to provide information, such as:<br />

1.Your personal data (Employee Number, Social Security<br />

Number, etc.);<br />

2. Your supervisor’s contact information;<br />

3. The start date <strong>of</strong> your disability;<br />

4. The day you last worked; and<br />

5. Your physician’s name and contact information.<br />

Step 2: Have your Health Care Practitioner call Sedgwick<br />

immediately between 8:00 a.m. and 5:00 p.m., Monday<br />

through Friday, at: 1-800-786-8600<br />

or contact them on the Internet, at:<br />

www.SedgwickCMS.com/Calabasas<br />

Sedgwick will ask your Health Care Practitioner to provide:<br />

1. The start date <strong>of</strong> your illness or disability;<br />

2. An estimate <strong>of</strong> your return-to-work date; and<br />

3. Any modified duties that you may be able to perform.<br />

The Sedgwick representative will enter the information received from you<br />

and your Health Care Practitioner directly into their computer system and<br />

open your claim file.<br />
