Primary Care <strong>Provider</strong> (PCP)<br />
Each member must choose a PCP. They are listed by county in this directory.<br />
Specialty Care<br />
This directory also has a list of specialty doctors. When you need special care or<br />
treatment, your PCP can help you find the right provider.<br />
Vision<br />
For vision, contact Avesis at 1-866-522-5923. They will help you with your needs for<br />
yearly eye exams and glasses. A listing of doctors to choose from is included in this<br />
directory.<br />
Dental<br />
Call Doral Dental at 1-800-205-4715 to select a dentist. They will do checkups and<br />
cleanings. They also do X-rays and fillings. Check this directory for a listing of doctors.<br />
Mental Health and Substance Abuse<br />
For behavioral health, call Magellan Health Services at 1-800-424-5412. Check this<br />
directory for a listing of doctors.<br />
Laboratories<br />
When your doctor sends you for lab services, you should use one of the labs in this<br />
directory. Check for the one that is closest to you.<br />
Pharmacies<br />
Prescriptions should be filled at the pharmacies in this directory. Look for one that is<br />
closest to you.<br />
Hospitals<br />
Hospitals are listed by county. Find the hospital closest to you in case of an emergency.<br />
