REQTYP E Conditions:
1. DL form is required when ACT is V and ELT is C, otherwise DL is prohibited when ELT is A.
2. EU form is required when SC is CA or NV and ACT is R.
3. EU form is prohibited when SC is IL, IN, MI, OH, WI, MO, OK, KS, AR, TX or CT and ACT is R.
4. EU form is required when ACT is C and LNA is N or ACT is C and LNA is C and 2nd position of TOS is H.
5. When ACT is C, LNA is C and a DL form is present, an entry is required in one or more of the following fields: ACCOUNT FEATURE on the LSR form
or HA on the Hunt Group Information form or PIC, LPIC, BA, FEATURE or OTN on the Resale form.
6. HGI and DL forms are prohibited when ACT is C, ADET is G.
7. RESALE form is prohibited when ACT is C, ADET is G, otherwise required.
8. EU form is required when ACT is C, ADET is G.
