Recommendations<br />
I recommend that Dr B take <strong>the</strong> following action:<br />
• Apologise to Mr <strong>and</strong> Mrs A in writing. This apology is to be sent to <strong>the</strong><br />
Commissioner <strong>and</strong> will be forwarded to Mr <strong>and</strong> Mrs A.<br />
• In <strong>the</strong> event that she returns to obstetric practice, undergo fur<strong>the</strong>r training on <strong>the</strong><br />
interpretation <strong>of</strong> CTG tracings.<br />
Follow-up actions<br />
• A copy <strong>of</strong> my final report will be sent to <strong>the</strong> Medical Council <strong>of</strong> New Zeal<strong>and</strong>, <strong>the</strong><br />
Royal Australian <strong>and</strong> New Zeal<strong>and</strong> College <strong>of</strong> Obstetricians <strong>and</strong> Gynaecologists,<br />
<strong>and</strong> <strong>the</strong> Accident Compensation Corporation.<br />
• A copy <strong>of</strong> my final report, with details identifying <strong>the</strong> parties removed, will be<br />
placed on <strong>the</strong> <strong>Health</strong> <strong>and</strong> <strong>Disability</strong> Commissioner website, www.hdc.org.nz, for<br />
educational purposes.<br />
