3.2.3. <strong>Postpartum</strong> Clinics<br />

POSTPARTUM AND POSTNATAL GUIDELINES<br />

Seven <strong>of</strong> the twelve facilities with active maternity services <strong>of</strong>fer breastfeeding<br />

support and consultation through an on-site breastfeeding/postpartum clinic, usually<br />

located on or near the maternity unit. The majority <strong>of</strong> these clinics are open Monday<br />

to Friday during ‘day-time’ hours and are staffed by a lactation consultant. Two <strong>of</strong><br />

these clinics provide all breastfeeding women and their infants with an appointment<br />

to return within 48 hours <strong>of</strong> discharge.<br />

Women looking for ‘after-hours’ consultation regarding a self-care or infant care<br />

issue or a breastfeeding problem <strong>of</strong>ten call or visit the maternity unit or the hospital<br />

emergency/outpatient department.<br />

3.2.4. Mother and Baby Leave Early (MABLE) <strong>Program</strong><br />

The MABLE <strong>Program</strong> is <strong>of</strong>fered to healthy mothers and babies who are discharged<br />

from the IWK Health Centre prior to 48 hours following vaginal birth, or 72 hours<br />

following cesarean section. The woman must reside within a specified geographical<br />

location - generally defined as being within a 30 minute drive <strong>of</strong> the IWK Health<br />

Centre. The <strong>Program</strong> is also available to women who have had babies with special<br />

care needs (e.g. infants admitted to the Special <strong>Care</strong> Nursery), women who have<br />

placed their children for adoption and those who have had stillbirths. Nursing<br />

assessments and family wishes are <strong>of</strong>ten instrumental in initiating admission to the<br />

<strong>Program</strong>. When all the criteria apply, admission to the <strong>Program</strong> remains voluntary.<br />

The initial contact is made by telephone within 24 hours <strong>of</strong> discharge from the IWK<br />

and a visit time is arranged, based on identified need.<br />

Since November 2001, a nurse does not automatically visit all women who enter the<br />

MABLE <strong>Program</strong>. All mothers are still contacted by telephone but, if a visit is<br />

deemed unnecessary, women are made aware <strong>of</strong> the services available through<br />

Public Health Services and the IWK Health Centre’s Mother and Baby Clinic. The<br />

Mother and Baby Clinic is open to all women who give birth at the IWK Health<br />

Centre, regardless <strong>of</strong> whether they were enrolled in the MABLE <strong>Program</strong>. This clinic<br />

is open seven days a week from 11:00 am to 6:00 pm, operating on both scheduled<br />

and ‘drop-in’ appointments.<br />

4. WORKING GROUP ASSUMPTIONS<br />

The Working Group reviewed the information available to them and suggested a<br />

series <strong>of</strong> guidelines to enhance and support the provision <strong>of</strong> quality care to Nova<br />

Scotian women, their babies and their families in the immediate (six weeks)<br />

postpartum period. The Group viewed postpartum care as one stage within the<br />

larger continuum <strong>of</strong> maternal/child care which, in common with all health programs,<br />
