SECTION ‘C’: Status of Application –To be completed by the Elective Office

You are required to send in your confirmation within 15 days from the date of this approval along with following:
- Elective Confirmation Form (EV-II form is enclosed)
- AKU Identity Card Form (form is enclosed)
- AKU Hostel Request Form (form is enclosed, if applicable. For status of hostel accommodation please see
section „D‟ below)
- 4 photograph (size 1.25 in x 1.5 in)
- 2 copies of CNIC
- A bank draft / pay order of Rs. 2,000/- in the name of “Aga Khan University” as elective registration fee.
On the first day of electives, please report to the Elective Office located in the School of Nursing, Aga Khan
University at 8:30 am.
Evaluation Form: A letter verifying your placement and performance will be provided to you by the elective office
upon receipt of clearance and ID card. It remains the discretion of your supervising faculty to complete the
evaluation form of your institution. Further, student will be required to follow-up for their Letter of
Recommendation with supervising faculty member.
Note:
Elective offer will stand CANCELLED for selected students not joining on approved dates.

SECTION ‘D’: Status of Accommodation – To be completed by the Hostel (if applicable)

- A bank draft / pay order of Rs. 11,250/- in the name of “Aga Khan University” as hostel fee.
- A bank draft / pay order of Rs. 10,000/- in the name of “Aga Khan University” as security deposit.

* Off-campus hostels are located at nearby vicinity (within 5 km radius) of main campus. Shuttle service is
available for pick and drop.
