ACCREDITATION/credit DESIGNATION<br />
Baylor College of Medicine is accredited by <strong>the</strong> Accredit<strong>at</strong>ion Council for Continuing<br />
Medical Educ<strong>at</strong>ion to provide continuing medical educ<strong>at</strong>ion for physicians.<br />
Baylor College of Medicine design<strong>at</strong>es this live activity for a maximum of 12.25 AMA PRA<br />
C<strong>at</strong>egory 1 Credits. Physicians should claim only <strong>the</strong> credit commensur<strong>at</strong>e with <strong>the</strong> extent<br />

activity st<strong>at</strong>us and space availability. The O<strong>CME</strong> reserves <strong>the</strong> right to limit <strong>the</strong> number of<br />
participants in an activity and will not be responsible for any expenses incurred by an<br />

Cancell<strong>at</strong>ion/Refund Policy<br />
Requests for registr<strong>at</strong>ion refunds must be in writing and received by <strong>the</strong> O<strong>CME</strong> <strong>at</strong> least ten<br />
business days before <strong>the</strong> activity begins. The d<strong>at</strong>e <strong>the</strong> request is received by <strong>the</strong> O<strong>CME</strong> will<br />
be considered <strong>the</strong> cancell<strong>at</strong>ion d<strong>at</strong>e. Requests received after <strong>the</strong> refund deadline will not be<br />
processed. Cancell<strong>at</strong>ions are subject to a $50 administr<strong>at</strong>ive fee deducted from <strong>the</strong><br />
registr<strong>at</strong>ion fee paid to cover guarantees and o<strong>the</strong>r expenses. Requests should be mailed to<br />
<strong>the</strong> O<strong>CME</strong> or faxed to 713.798.7955.<br />
The O<strong>CME</strong> reserves <strong>the</strong> right to cancel activities, not less than ten business days before<br />
<strong>the</strong> scheduled d<strong>at</strong>e, if extenu<strong>at</strong>ing circumstances make it necessary. Registrants will be<br />
notified <strong>at</strong> <strong>the</strong> contact number indic<strong>at</strong>ed on <strong>the</strong> registr<strong>at</strong>ion form followed by written<br />
notific<strong>at</strong>ion. If an activity is cancelled, <strong>the</strong> O<strong>CME</strong>’s liability is limited to <strong>the</strong> registr<strong>at</strong>ion fee<br />
paid. Note: If payment is made by check, a social security number is required to process<br />
