ARDS
Exudative phase – progression of injury to interstitium and
alveolar cells, capillary leak, formation of hyaline
membranes. Edema leading to decreasing lung compliance
and intrapulmonary shunting.
Proliferative (regenerative) phase – gradual reduction of
inflammatory process, proliferation of type II
pneumocytes. Fibrosis starts. Continued interstitial
edema.
Fibrotic phase – deposition of collagen in interstitial spaces
and in collapsed alveolar spaces.
Self limiting with rapid recovery or if severe and
prolonged leads to largely irreversible lung injury.
