No. Members and or their qualified dependents availing of the PCBI package need not<br />

pay any amount since services included in the package are paid to the Provider through the<br />

PFPR. If the member/dependent is referred for diagnostic services to another facility, the<br />

PCB provider shall pay for that service as provided in the MOA with Referral facility.<br />

14. What are my roles and responsibilities as an entitled member/dependent?<br />

The following are your roles and responsibilities as an entitled member/dependent of<br />

PCB1.<br />

• Bring at all times your Phil<strong>Health</strong> Number Card or Identification Card.<br />

• Update your Member Data Record (MDR) for any change in personal information such<br />

as change in civil status or addition of a new dependent.<br />

• Inform your LHIO if there will be subsequent changes in your address<br />

• Request for a replacement in case of loss of Phil<strong>Health</strong> Number Card or Identification<br />

Card.<br />

• Ensure that you promptly and regularly pay your contributions (for Organized Groups<br />

and Overseas Workers Program Members) to avoid suspension of <strong>benefit</strong>s.<br />

• Be familiar with the services which one can avail of in PCB1<br />

• Cooperate with the health<strong>care</strong> provider in terms of enlistment and profiling procedure<br />

• Sign a waiver if procedures to be done are in conflict with one’s own personal beliefs<br />

• Be aware of amendments and updates on Phil<strong>Health</strong> policies and <strong>benefit</strong>s schedule.<br />

• Seek clarification from any Phil<strong>Health</strong> office on any unclear policy or guideline.<br />

• Report at once to Phil<strong>Health</strong> any health<strong>care</strong> facility that fails, without valid reason, to<br />

accommodate a Phil<strong>Health</strong> member who wishes to avail of <strong>benefit</strong>s.<br />

• Report at once to Phil<strong>Health</strong> any fraudulent transaction that you know about.<br />

• Observe and comply with Phil<strong>Health</strong> rules and regulations as there are offenses in its<br />

Implementing Rules and Regulations<br />

ON PCB 1 PROVIDERS<br />

1. What kind of health <strong>care</strong> facility will qualify as Primary Care Benefit (PCB) Provider?<br />

Any government health facility that has the capacity and human resources to deliver the<br />

PCB I shall qualify as Primary Care Benefit (PCB) provider. These include, but are not limited<br />

to government‐owned health stations/health centers/rural health units (BHS/HCs/RHUs) and<br />

Out‐Patient departments of government hospitals, including those previously authorized as<br />

OPB Providers.<br />

2. What happens to the current OPB participating health facilities?<br />
