staff member as a result of incorrect assembly/disposal, the area the<br />
sharps box originated from can be immediately identified. Identifying<br />
the origin of the sharps box <strong>and</strong> its contents is imperative to assist in<br />
the immediate risk assessment process carried out following a sharps<br />
injury <strong>and</strong> also to ensure that staff who incorrectly assembled/<br />
disposed of the sharps box can receive education on the correct<br />
procedures to follow.<br />
In the preparation room a number of temporary closures on sharps<br />
boxes was open <strong>and</strong> it was noted that sharps boxes with integral trays<br />
were stored on the lower shelf of the procedure trolleys for transport to<br />
the bedside. Brackets were not attached to the trolleys to secure the<br />
boxes. With little effort <strong>and</strong> improvement in staff practice, staff could<br />
achieve full compliance in this st<strong>and</strong>ard.<br />
Recommendations<br />
5. The trust should ensure that waste bins are kept clean, in<br />
good repair <strong>and</strong> replaced as appropriate.<br />
6. The trust should monitor the implementation of its policies<br />
<strong>and</strong> procedures in respect of the management of waste <strong>and</strong><br />
sharps to ensure that safe <strong>and</strong> appropriate practice is in<br />
place.<br />
