must be reported to the appropriate hiring authority for proper disposition. A conflict <strong>of</strong> interest is<br />

created when an individual evaluates or supervises or has decision making power affecting another<br />

individual with whom he or she has an amorous or sexual relationship. Moreover, such relationships,<br />

even when consensual, may be exploitative and imperil the integrity <strong>of</strong> the work or education<br />

environment.<br />

Training – The <strong>University</strong> <strong>of</strong> <strong>Florida</strong> provides online sexual harassment prevention training to promote<br />

and increase awareness and prevention <strong>of</strong> sexual harassment. New employees are required and<br />

expected to complete the tutorial within their first 30 days <strong>of</strong> employment. To do so, sign onto the<br />

myUFL system and navigate to My Self Service, Training and Development, Online Learning,<br />

“Preventing Sexual Harassment.” This important online training includes information about the<br />

<strong>University</strong>’s Sexual Harassment Policy as well as our roles in and responsibilities for keeping our<br />

workplace free from sexual harassment.<br />

COMMISSION ON DENTAL ACCREDITATION COMPLAINT POLICY<br />

The Commission on Dental Accreditation will review complaints that relate to a program's compliance<br />

with the accreditation standards. The Commission is interested in the sustained quality and continued<br />

improvement <strong>of</strong> dental and dental-related education programs but does not intervene on behalf <strong>of</strong><br />

individuals or act as a court <strong>of</strong> appeal for treatment received by patients or individuals in matters <strong>of</strong><br />

admission, appointment, promotion or dismissal <strong>of</strong> faculty, staff or students. A copy <strong>of</strong> the appropriate<br />

accreditation standards and/or the Commission's policy and procedure for submission <strong>of</strong> complaints<br />

may be obtained by contacting the Commission at 211 East Chicago Avenue, Chicago, IL 60611-2678 or<br />

by calling 1-800-621-8099 extension 4653or visit the ADA website at:<br />

CERTIFICATE OF COMPLETION<br />

A certificate <strong>of</strong> advanced education training will be issued to a resident on the recommendation <strong>of</strong> the<br />

college’s appropriate chair and program director only after satisfactory completion <strong>of</strong> service and<br />

educational requirements and fulfillment <strong>of</strong> all other obligations and debts. This includes completion<br />

<strong>of</strong> medical/dental records, appropriate consent for use <strong>of</strong> any patient records after graduation, a<br />

checklist completed for each program co-signed by program director and associate dean for advanced<br />

and graduate education and return by the resident <strong>of</strong> State <strong>of</strong> <strong>Florida</strong> property, as well as property <strong>of</strong><br />

any affiliated institution.<br />

Graduate degree-seeking students will only be awarded the certificate upon completion <strong>of</strong> the<br />

requirements for the Master <strong>of</strong> Science degree.<br />
