2. If the organization has had a contract terminated <strong>for</strong> default during the past five (5)<br />
years, submit full details surrounding the termination including the nature of the<br />
default and the terminating party’s name, address and telephone number<br />
3. Provide the full company or corporate name, address and phone number along with<br />
the project contact person full name<br />
4. Provide a list of subcontractors to include their full company or corporate name,<br />
address, telephone number and contact person<br />
E. References<br />
Provide a list of at least three (3) clients <strong>for</strong> whom a similar service has been provided and dates when the<br />
services were provided. Include client name, address, telephone number, facsimile number and person to<br />
contact.<br />
2. Right to Audit<br />
A. The <strong>County</strong> shall have access to the Contractor’s facilities, to all necessary records, and shall be provided<br />
adequate and appropriate workspace, in order to conduct audits in. The <strong>County</strong> shall give the Contractor(s) a<br />
10-day advance notice of intent to audit.<br />
B. If an audit discloses overcharges (of any nature) by the Contractor(s) to the <strong>County</strong> in excess of five percent<br />
(5%) of the total contract billings, the actual cost of the <strong>County</strong>’s audit shall be paid by the Contractor(s).<br />
3. Additional Services<br />
<strong>Richland</strong> <strong>County</strong> may add to the Scope of Services or make changes in the Scope of Services with any<br />
services of a similar nature to those specified in the Scope of Service of this <strong>Request</strong> <strong>for</strong> <strong>Proposal</strong> as<br />
mutually agreed upon in writing at a price mutually agreed upon.<br />
4. Licenses and Permits<br />
The Contractor shall secure, pay <strong>for</strong> and maintain all permits, governmental fees and licenses which are<br />
legally required <strong>for</strong> the proper execution and completion of the work outlined, <strong>for</strong> the life of the<br />
agreement.
