Applications must include a maximum two-page, minimum 10-point font size, written summary of the project. This<br />

Nominations may also include letters of support, photos, news clippings, or visual or digital media illustrating the<br />

achievement and are encouraged. For Distinguished Contributors to Arizona Planning, please include the names,<br />

mailing addresses, phone numbers, and e-mails of two references who may be contacted for additional in<strong>form</strong>ation.<br />

A Complete Nomination Application<br />

Please follow these instructions to the letter. Twelve (12) copies of a complete Nomination <strong>application</strong> are required.<br />

Incomplete <strong>nomination</strong>s will not be considered. A complete <strong>nomination</strong> <strong>application</strong> must include:<br />

1. Hard copy (e.g., printed on paper), completed <strong>nomination</strong> <strong>application</strong> <strong>form</strong>.<br />

2. Hard copy (e.g., printed on paper) maximum, two-page narrative.<br />

3. Digital (paper copy is permitted, digital copies are preferred) supplemental supporting materials (letters, newspaper<br />

clippings, photographs) for the Distinguished Contributor to Arizona Planning categories, remember to<br />

include the references. Please provide all digital files as a *.pdf.<br />

4. Digital (paper copy is permitted, digital copies are preferred) project documents, plans, reports, studies being<br />

nominated. Please provide all digital files as a *.pdf.<br />

5. All submittals must be secured and organized within re-sealable or closable folders or other binder. APA<br />

Arizona is not responsible for assembling or copying submission materials.<br />

6. Submit one separate CD or DVD with three-to-five PowerPoint (*.ppt) slides or digital photos (*.jpg, *.pdf,<br />

*.png) of the <strong>nomination</strong>. These images are to illustrate the significance of the submission and may be reproduced<br />

by the chapter for promotional purposes and used during the awards ceremony. Images shall be:<br />

Not more than 120 dpi (96 dpi preferred) resolution; and<br />

Sized to a maximum of 1024 pixels wide for landscape images; or<br />

A maximum of 768 pixels high for portrait images.<br />

All submissions become the property of APA Arizona.<br />

Nomination materials will not be returned.<br />

Committee policies prohibit any communication with any committee member on behalf of a <strong>nomination</strong>.<br />

Eligibility<br />

Nominators may submit more than one entry per category. Each submission may be entered in only one<br />

award category. If entering the same entry in multiple categories, the additional <strong>application</strong>s are treated as<br />

separate <strong>nomination</strong>s and the appropriate fees and copies are needed.<br />

Any plan, project, or program entered must have been adopted or accepted within the last five years, except<br />

for the historic <strong>planning</strong> landmark category which must be a minimum of 25 years old.<br />

Student efforts submitted by public or private practicing planners, public agencies or municipalities must be<br />

entered in an appropriate awards category other than Student Planning. The Student Planning category is<br />

reserved for classroom projects.<br />
