Durable Medical Equipment includes, but is not limited to, wheelchairs, crutches, oxygen systems and related supplies,<br />

disposable medical supplies (diabetic, ostomy, etc), orthotics, and prosthetics.<br />

Home Health/Home Infusion care can either be a licensed facility or medical personnel that can provide health care services<br />

in the home. Services include but are not limited to nursing visits for infusion services or wound care.<br />

Laboratory means diagnostic testing, including routine blood and urine analysis, anatomic pathology, surgical pathology, and<br />

specialty or esoteric services.<br />

Specialty Pharmacy Provider provides specific drug products directly to customers or physicians.<br />

Women's Health is limited to home uterine monitoring services.
