i. <strong>DISTRICT</strong> <strong>HEAD</strong> <strong>QUARTER</strong> <strong>HOSPITAL</strong> NAROWAL.<br />

21-03-2009 at 08:30 AM.<br />
1. Location is on the main road and outlook of Hospital is satisfactory.<br />
2. Signboard / Direction Board was displayed at the by pass of Narowal City as well<br />
as in front of DHQ Hospital.<br />
3. DHQ Hospital was established in 1991. Originally it was a dispensary,<br />
established in 1927 which was upgraded as THQ Hospital Narowal in 1941.<br />
4. DHQ Hospital has capacity of Forty Beds.<br />
5. New Campus of DHQ Hospital with Additional Facility of 125 Beds through<br />
provincial ADP Scheme is under way and 40% work is completed.<br />
6. Dr. Muhammad Abbas Qari, Medical Superintendent of DHQ Hospital, Narowal is<br />
holding the office since 31-05-2008, who was present in his office.<br />
7. Organogram, other Statistics / Informations of the District & Up to date Vacancy<br />
Position was displayed in MS Office.<br />
8. Following Doctors were yet to reach in Hospital for duty:<br />
• Dr. Shafique Ahmad, Surgeon (Late)<br />
• Dr. Imrana Imran, Gynecologist (Late)<br />
• Dr. Muhammad Sarfraz, Dental Surgeon (Late)<br />
• Dr. Basarat Ellahi, Homeo Doctor (Late)<br />
• Dr. Amanat Ali, Eye Surgeon reached for duty in DHQ at 08:40 AM.<br />
• Dr. Muhammad Idrees, Physician was on authorized leave.<br />
9. Utilities are available like Electricity, Telephone, Sewerage & Proper Water<br />
Supply System with turbine and water reservoir.<br />
10. Cash Book was updated. Verification of Purchee Fee Deposits from District<br />
Treasury was also updated.<br />
11. Against One Hundred & Seven sanctioned posts of various categories, Twenty<br />
Two posts with following details were vacant:-<br />
APMO (Male + Female) (7), Principal Dental Surgeon (1), Gynaecologist (2),<br />
Anaesthesialogist (1), ENT Specialist (1), Orthopedic Surgeon (1), Pharmacist<br />
(1), MO (1), WMOs (2), Physiotherapist (1), Pathologist (1), Radiologist (1) &<br />
Junior Clerk (1).<br />
Medical Superintendent was requested to take action as per policy and<br />
rules to fill the vacant posts.<br />
12. Existing Building of DHQ needs routine repair.
13. Room Place for offices is insufficient. Furniture is also required.<br />
14. Toilets, Waiting area and Wards are insufficient, keeping in view the patients turn<br />
out.<br />
15. Laboratory of Hospital is not up to the standards and yardstick of DHQ. Lab<br />
Assistant & Lab Attendant were not observing necessary precautionary<br />
measures while getting blood samples of patients and they were without proper<br />
gloves.<br />
Medical Superintendent was requested to ensure the observance of all<br />
required precautionary measures.<br />
16. During visit of MNCH Centre, LHWs informed that monthly salary is not being<br />
disbursed to them regularly since January 2009.<br />
Medical Superintendent was requested to take up the issue with Provincial<br />
Coordinator of National Program PHC & FP to resolve the issue.<br />
17. During visit of main medical store, it was revealed that there is shortage of<br />
Antibiotic Tab / Capsules, which is being met through LP but major supply for<br />
which order has been placed is awaited.<br />
18. Dr. Shafique Ahmed, Surgeon requested for the provision of Three Lapratomy<br />
Set, Vascular Sets for AV Fistula & Emergency Kit. OT Light of Operation<br />
Theatre was also out of order.<br />
Medical Superintendent was requested to take action as per policy.<br />
19. Hospital Management is facing problems of funds under following heads:<br />
Repair of Vehicles, Others, Medicines, X-Ray Films & Lab Materials.<br />
EDO (Health) Narowal is requested to look into this and facilitate the<br />
hospital administration. M S was also requested to take necessary action<br />
while finalizing second statement of Excess and Surrender.<br />
20. DHQ Narowal is included in the list of Less Attractive DHQ Hospitals but M S is<br />
not being accommodated with any additional incentive.<br />
EDO (Health) Narowal is requested to proceed further as per policy.<br />
21. Only Dental Unit of Hospital is Non-Functional and Unserviceable.<br />
EDO (Health) Narowal is requested to look into this and go for proper<br />
replacement to ensure the delivery of services in Dental Unit of DHQ.<br />
22. Facility of Dialysis Unit is available in DHQ with Four Machines. Only two patients<br />
were served since January 01, 2009 till the date of visit.
ii. TEHSIL <strong>HEAD</strong> <strong>QUARTER</strong> <strong>HOSPITAL</strong> SHAKAR GARH <strong>DISTRICT</strong> NAROWAL<br />
Date of visit<br />
21-03-2009 at 1330 HRs.<br />
1. Location is on the main road and outlook of Hospital is satisfactory.<br />
2. Signboard / Direction Board was displayed at by pass of city and in front of<br />
hospital.<br />
3. THQ Hospital was established in 1962.<br />
4. Total Bed Capacity of THQ Hospital is 40 Beds and only 20 Beds are in working<br />
condition.<br />
5. Dr. Rafique Ahmed, Medical Superintendent was on Umrah leave and Dr.<br />
Muhammad Latif Afzal was looking after the office of M S THQ Hospital<br />
Shakargarh.<br />
EDO (Health) Narowal was requested to send copies of orders in this<br />
regard.<br />
6. During visit of THQ Hospital, it was revealed that Dr. Naeem Ullah Khan, Peads<br />
Specialist and Dr. Muhammad Asad Ullah Khan, Surgeon was on Unauthorized<br />
Leave. Their staff presented such leave applications in which date was not<br />
specifically mentioned for which leave was required. These applications were<br />
never presented before the Medical Superintendent to whom these were<br />
addressed for grant of leave. Dr. Naeem Ullah Khan left duty place / station for<br />
Lahore without seeking proper permission. The non-serious attitude and nonobservance<br />
of official conduct on the part of both doctors was speaking a lot.<br />
(Applications are placed at Annex-A & B for perusal).<br />
EDO (Health) Narowal was briefed about this and he informed that both<br />
these doctors are habitual of this sort of mis-conduct and non-serious<br />
attitude.<br />
7. Dr. Muhammad Latif Afzal, who was looking after the office of Medical<br />
Superintendent, THQ Hospital Shakar Garh, was using Writing Pad of his Private<br />
Hospital which has no mentioning of his official capacity, although he was sitting<br />
in the office of Government Hospital. (Copy of the same is placed at Annex-C for<br />
perusal).<br />
EDO (Health) Narowal was briefed about this with the request to take<br />
appropriate action to ensure discipline.<br />
8. Fluid Dextro 5% (1000cc) & Haemacele were out of stock.<br />
9. Shortage of Antibiotic Tabs / Syrups / Injections, Analgesic Tabs / Injections &<br />
Scabies Lotions was also observed in the main store of Medicines.<br />
EDO (Health) Narowal was briefed about this situation with the request to<br />
ensure the un-interrupted supply of medicines.
10. Building of THQ is old one with problems of Sewerage and Water Supply.<br />
11. Offices were also insufficient, keeping in view the requirement.<br />
12. Construction of four new residences is in process.<br />
13. X-Ray Film was not available in THQ Hospital and patients were being referred<br />
to Private Hospitals for X-Ray.<br />
EDO (Health) Narowal was briefed about this with the request to take<br />
corrective measure on urgent basis.<br />
14. Emergency Ward was without proper Toilet Facility. Acting M S also requested<br />
for the major repair of Mortuary which was in poor condition.<br />
15. Water Supply Scheme of THQ Hospital is to be made functional on urgent basis<br />
as requested by Doctors during visit.<br />
EDO (Health) Narowal was briefed about this with the request to look into<br />
this and coordinate with concerned department.<br />
16. Out of Two Ambulances, one was non-functional and repairable.<br />
17. EPI Centre of THQ Hospital was functional and vaccination / immunization of<br />
70 / 80 children and women were being carried out.<br />
18. Three Posts of Specialists i.e. Two Gynecologist and One Physician were<br />
vacant.<br />
19. Six Posts of MOs / WMOs were also vacant.<br />
EDO (Health) Narowal was requested to take action for filling of these<br />
vacant posts as per policy.
