Introduction: Katz and Stirrat self-administered hand diagram <strong>for</strong> <strong>the</strong> diagnosis <strong>of</strong> carpal tunnel syndrome<br />

(CTS) is in use in <strong>the</strong> pre-hospital community setting. <strong>The</strong> reliability <strong>of</strong> this diagnostic tool is revisited<br />

<strong>here</strong>.<br />

Material and Methods: Twenty-five patients with CTS (confirmed by electroneurophysiological studies),<br />

25 with o<strong>the</strong>r hand or wrist pathologies and 25 normal individuals were prospectively evaluated by <strong>the</strong><br />

self-administered hand diagram. Sensitivity, specificity, positive and negative predictive values were<br />

calculated. <strong>The</strong> diagrams were blindly scored by two experienced hand surgeons on two different settings<br />

three weeks apart. Inter-observer and intra-observer reliability were assessed.<br />

Results: <strong>Hand</strong> diagram had a sensitivity <strong>of</strong> 58.7%, specificity <strong>of</strong> 80%, positive predictive value <strong>of</strong> 89.9%<br />

and negative predictive value <strong>of</strong> 39.2%. 95% correlation interval was 0.33–0.65 <strong>for</strong> intra-observer<br />

measurement. Kappa value <strong>of</strong> 0.241 showed a fair agreement in between <strong>the</strong> two observers.<br />

Conclusion: <strong>Hand</strong> diagram has a low sensitivity and negative predictive value. <strong>The</strong> inter-observer<br />

agreement is fair and intra-observer repeatability is poor. This tool is nei<strong>the</strong>r sensitive enough nor reliable<br />

to be used as a tool in <strong>the</strong> assessment <strong>of</strong> CTS. A thorough history and appropriate clinical examination is<br />

more accurate <strong>for</strong> <strong>the</strong> diagnosis <strong>of</strong> CTS.<br />
