UNIFORM ORDERING INSTRUCTIONS Welcome to Cintas! Cintas is excited to be the uniform supplier for KeepRite Heating and Cooling Products. Please read these instructions carefully before completing this order form. 1. Please print legibly. Your order will be processed more quickly if the order form is completed as indicated below. 2. Complete the "Ship To" address information including phone and fax number on the order form. (No P. O. boxes please). 3. If you are paying for your order by check, money order, VISA, MasterCard, American Express, or Discover Card, please complete the "Prepaid Information" on the order form. PLACING YOUR ORDER 1. Complete the sizing information for each garment being ordered. See back of order form for size information. 2. Mail, fax (513.459.1532) or phone (877.368.0843) your order to Cintas. If you are submitting an order for the first time, a credit authorization will be processed for the first order placed. Cintas reserves the right to request prepayment of any order pending credit approval. (This process does not apply if paying for the order at time of purchase.) Please allow at least 10 working day to process and ship your order form Cintas. After you receive your garments, please try time on to make sure they fit prior to laundering. If returns are necessary, please allow the instructions accompanying your order.
