2. Air Waybill<br />
(UPS will NOT SHIP <strong>Infectious</strong> Substance, Category A!!)<br />
a. <strong>Infectious</strong> substance, Category A<br />
i. For an infectious substance, under “H<strong>and</strong>ling Information” on the Air waybill,<br />
state “Dangerous Goods per attached Shipper’s Declaration for Dangerous<br />
Goods” or “Dangerous Goods per attached Shipper’s Declaration for Dangerous<br />
Goods Cargo Aircraft only”.<br />
b. <strong>Biological</strong> substance, Category B<br />
i. The name, address <strong>and</strong> phone number of the responsible person must be on this<br />
document if it is not on the box.<br />
NOTE: The phone number of the responsible person does not need to be<br />
CHEMTREC. The number should be available during regular business hours in case<br />
of questions or problems.<br />
ii. Dry Ice must appear as the following: (Dry Ice UN 1845 3kg)<br />
iii. The “Nature <strong>and</strong> Quantity Goods” box must show the text “BIOLOGICAL<br />
SUBSTANCE, CATEGORY B” <strong>and</strong> UN 3373.<br />
c. Exempt specimens<br />
i. The “Nature <strong>and</strong> Quantity Goods” box must show the text “Exempt animal<br />
specimen” or Exempt human specimen”.<br />
d. Dry Ice<br />
e. Dry Shipper<br />
i. The “Nature <strong>and</strong> Quantity Goods” box must show the text “Dry Ice UN 1845 x<br />
kg”.<br />
i. The “Nature <strong>and</strong> Quantity Goods” box must show the text “not restricted” <strong>and</strong><br />
“A152”.<br />
f. Liquid Nitrogen<br />
i. The “Nature <strong>and</strong> Quantity Goods” box must show the text “UN1977 Nitrogen,<br />
refrigerated liquid”.<br />
