Explain the rationale for involvement of animals. • *In addition to the narrative, you must also complete and submit forms to the Sutter Institute for Medical Research’s Animal Care and Use Committee (ACUC). The forms are available through the SIMR Research Office. It is recommended that investigators meet with the SIMR Veterinarian prior to submitting forms to the ACUC for review. Include a copy of the completed forms with your proposal. It is not necessary to obtain ACUC approval prior to submitting your proposal, but ACUC approval must be obtained prior to commencement of the project. Dissemination of the results Describe how you plan to use the results of the study. Discuss where you plan on publishing the results of your findings. *Budget BUDGET AND BUDGET JUSTIFICATION 1. Prepare a budget for each year of the study using the spreadsheet provided. The spreadsheet will automatically calculate the totals for the Summary Budget Page. 2. When preparing your budget, include only those costs directly associated with the objectives and activities of the research project. Patient care costs can only be included if they are associated with the study. Do not include any indirect costs except on the line indicated at the bottom of the form. Following is an explanation of the budget page. Personnel Consultants Equipment Supplies • Name and Role on Project: List each position that will be involved with the project, including research assistants, technical staff, and clerical support. • Appointment (months): Length of time, in months, that the person will be working on the project. This should be no longer than 12 months for each budget form submitted, exclusive of the summary budget form. • % Effort on Project: The total number of hours each person is expected to work on the project per year divided by 2080 (total number of working hours in a year). • Institutional Salary: If this person is currently employed by Sutter Health, list the annual salary (not including benefits). • Salary Requested: Multiply the Base Salary by the % Effort; divide by 12 to get the monthly salary; and multiply by time, in months, to calculate the Project Salary Costs • Benefits: Includes employer taxes, medical benefits, and other Sutter Health employer paid benefits. Unless otherwise indicated, use 33% of the salary requested for each position’s fringe benefits. • Total: Add Project Salary and Fringe Benefits columns for each position and enter the totals here. • Indirect rate: The indirect rate for SIMR funded projects is 25%. This will be automatically calculated in the final cost of the grant. • List the name of any non-Sutter employees who will be employed for this project each year and the total cost for their services. • Type in the total cost of all equipment purchased each year of the study. • Supplies include office supplies, postage, paper and other items needed to conduct the study.
