equired standards. A major factor was the abolition of crown immunity and<br />
the loss of protection previously afforded to <strong>hospitals</strong> from prosecution under<br />
environmental regulations.<br />
In 1997 new, higher environmental standards were <strong>in</strong>troduced, which could not<br />
be met by a number of the rema<strong>in</strong><strong>in</strong>g waste disposal facilities, and alternative<br />
methods of disposal had to be established. Trust chief executives set up a<br />
Cl<strong>in</strong>ical <strong>Waste</strong> Services Review Group (now the Cl<strong>in</strong>ical <strong>Waste</strong> Steer<strong>in</strong>g Group)<br />
which was successful <strong>in</strong> agree<strong>in</strong>g a cl<strong>in</strong>ical waste disposal contract for 22 trusts<br />
(out of 28) and two islands health boards <strong>in</strong> <strong>Scotland</strong>. The contract is based<br />
around a heat dis<strong>in</strong>fection process, recognised as one of the most<br />
environmentally acceptable methods of disposal, and is expected to save more<br />
than £6 million over ten years.<br />
The trusts <strong>in</strong> Greater Glasgow have a separate arrangement. They have worked<br />
collaboratively s<strong>in</strong>ce 1995 and are halfway through a contract which also<br />
ensures that waste is handled <strong>in</strong> an environmentally sound manner. The two<br />
ma<strong>in</strong> contracts are good examples of trust collaboration, and it is the Steer<strong>in</strong>g<br />
Group’s <strong>in</strong>tention to build on exist<strong>in</strong>g expertise to create a base for further work<br />
on waste <strong>management</strong> issues.<br />
The Cl<strong>in</strong>ical <strong>Waste</strong> Steer<strong>in</strong>g Group <strong>in</strong>cludes representatives from the Property<br />
and Environment Forum Executive, which is a recognised centre of expertise<br />
with<strong>in</strong> the NHS and has developed key documents <strong>in</strong>clud<strong>in</strong>g:<br />
■ ‘<strong>Scottish</strong> Hospital Technical Note 3’ (Management and disposal of cl<strong>in</strong>ical<br />
waste with def<strong>in</strong>itions and guidance on all healthcare waste issues)<br />
■ Greencode (a computerised <strong>management</strong> system for trusts cover<strong>in</strong>g<br />
environmental awareness).<br />
It is important for the NHS to keep waste <strong>management</strong> under review, s<strong>in</strong>ce the<br />
requirements of legislation cont<strong>in</strong>ue to develop; for example, from 1 January<br />
2002 trusts will have to replace bags with rigid pack<strong>in</strong>g for <strong>in</strong>fectious waste<br />
transported by road which is likely to <strong>in</strong>crease further the cost of dispos<strong>in</strong>g of<br />
cl<strong>in</strong>ical waste.<br />
The aims of this study were to:<br />
■ <strong>in</strong>vestigate whether waste is be<strong>in</strong>g handled safely and effectively <strong>in</strong> accordance<br />
with the regulations<br />
■ <strong>in</strong>vestigate whether waste is properly segregated, recycled wherever possible,<br />
and disposed of economically<br />
■ identify and promulgate good practice<br />
■ identify scope for potential sav<strong>in</strong>gs.<br />
The study was carried out <strong>in</strong> consultation with the Cl<strong>in</strong>ical <strong>Waste</strong> Steer<strong>in</strong>g<br />
Group, and this report is based on the f<strong>in</strong>d<strong>in</strong>gs of local audit <strong>in</strong>vestigations at<br />
21 trusts (out of 47) dur<strong>in</strong>g 1998/99. It also makes use of cl<strong>in</strong>ical waste data <strong>in</strong><br />
respect of all trusts for 1998/99 collected by the Property and Environment<br />
Forum Executive.<br />
This report will act as a basel<strong>in</strong>e report aga<strong>in</strong>st which the performance of the<br />
NHS <strong>in</strong> <strong>Scotland</strong> and <strong>in</strong>dividual trusts can be measured. It is our <strong>in</strong>tention to<br />
carry out a follow-up audit <strong>in</strong> about 18 months time to establish what action<br />
has been taken and what improvements have been achieved.<br />
