McCance-Katz, MD, University of California at San Francisco, San Francisco, CA According to estimates from the nation’s new HIV incidence surveillance system, there were approximately 56,300 new cases of HIV/AIDS diagnosed in the United States in 2006. Moreover, among the more than one million people living with HIV in the U.S., approximately one-fifth do not know they are infected. Recent studies have shown that fewer than half of U.S. drug abuse treatment programs are currently offering HIV testing on-site to their patients (Brown Jr. et al., 2006; Oser, Tindall, & Leukefeld, 2007; Pollack, D’Aunno, In Press; Strauss, Des Jarlais, Astone, & Vassilev, 2003). NIDA’s Clinical Trials Network conducted a study to evaluate the effectiveness of strategies (referral, on-site rapid testing, on-site rapid testing + counseling) to (1) increase HIV testing acceptance and receipt of results and (2) decrease HIV sexual risk behaviors. The target population was individuals receiving drug abuse treatment within community-based drug abuse treatment programs in the United States. 2,452 patients were screened in 12 drug abuse treatment programs (including three methadone programs) across the U.S. to identify patients who had not been HIV tested in the past 12 months. The screening data indicate that only 28% had been tested in the prior year, and one-fifth had never been tested. In this session, the main study findings will be presented and their implications for OTP practice discussed. The last presentation will summarize the issues related to medication-assisted treatment and HIV/AIDS. Drug users’ access to antiretroviral therapy might be complicated. Additional comorbidities might cause poorer outcomes for HIV/AIDS. It is important for the healthcare provider to understand and manage the complex drug interactions that are clinically significant. The presentation will provide a general overview and evidence-based recommendations. The 2009 General Accountability Office Report examined factors contributing to methadoneassociated deaths and found that use of methadone for pain management increased the drug’s prevalence, contributing to the rise in methadone-associated overdose deaths. These deaths and other adverse incidents led to a number of lawsuits and proposed legislation. Research supports the perspective that opioid addiction is a medical disorder that can be treated safely and effectively with medications when they are administered under conditions consistent with their pharmacological efficacy and accompanied by necessary supportive services such as psychosocial counseling, treatment for co-occurring 8 AATOD National Conference | Building Partnerships: Advancing Treatment & Recovery
disorders, medical care and vocational rehabilitation. Physicians, psychiatrists, clinical staff, nurses, and opioid treatment program directors, providers and staff need to understand how to minimize incidents such as patients driving under the influence of multiple substances, unsafe diversion of take-home medication, unsafe induction and reduction of medication, and dosing practices without consideration of clinically assessed severity of dependence and metabolism rates. This workshop is for healthcare and mental health professionals who are involved in the direct care of patients seeking help for opioid dependence. Conducted by experts in insurance, law, policy and daily clinical practices, this training will provide a framework for applying best practices and developing comprehensive risk management strategies to improve patient safety and eliminate transfer, reduce and manage the risks associated with medication assisted treatment for opioid dependence. Participants will have the opportunity to ask questions and enter into a dialogue with faculty and each other to expand their knowledge of current trends and develop skills in identifying and preventing risks.
