New Mobile Homes - Forsyth County Government

CHECKLIST
Please complete the entire application package, including:
□ Permit Details Form
□ Approved Plat of Property (2 copies 8½ x 11’’ required)
• Plat approved by Forsyth Planning and Community Development thru minor plat
process
• Previously approved plat (copy can be obtained at courthouse)
□ Site Plan
• Actual dimensions of property
• Size and location of building on property showing distance from property lines
• Driveway location
• Location of any existing structures on property
• Porch and deck located on mobile home
□ Contractor: All mobile home permits must be obtained by a licensed mobile home
installer.
□ Property Owner Authorization Form
□ Proof of Water
1. Water meter purchase, 2. Well
□ Affidavits
1. Contractor Affidavits – Required. (must be signed and submitted prior to applying
for building permit).
• Electrical Affidavit.
• Plumbing Affidavit. (will only be required if customer is required to have a
licensed plumber)
• Well Affidavit. (if water is provided by well provided by Planning office clerk)
2. Self Work Affidavit (if applicable – will be provided by Planning office clerk)
□ Mobile Home Placement Letter to verify zoning and acreage of property (provided by
planning office clerk to be carried to Tax Commissioner’s Office when applying for mobile
home decal.)
□ Ad Valorem Tax Receipt & Decal to be provided by Tax Commissioner’s Office (to replace a
mobile home a tax receipt showing paid taxes on old mobile home is required at time of
permit.)
□ Georgia Erosion & Sediment Control Certification Card – Required if 1 acre of land
disturbance. (Copy of card required)
□ Temporary Toilet / Exemption Form – Required by Forsyth County Health Department (will
be provided by Planning office clerk)
□ Septic Tank Permit – Required if on septic. Obtain at Forsyth County Health Department
(770-781-6909) prior to applying for building permit.
□ Fees – See Fee Schedule. Impact fees are required for all new finished space.
