Section 1 – Introduction<br />

This directory provides a list of your plan’s network<br />

providers. To get detailed in<strong>for</strong>mation about your<br />

health care coverage, please see your Evidence of<br />

Coverage (EOC).<br />

Your plan may cover additional benefits that are not<br />

covered by Original <strong>Medicare</strong>. Additional benefits<br />

may include vision, dental or hearing services. Note:<br />

You may have to use a specific provider <strong>for</strong> some<br />

additional benefits.<br />

Choosing a Primary Care Physician (PCP)<br />

You will have to choose one of our network providers<br />

who are listed in this directory to be your Primary<br />

Care Physician (PCP). The term “PCP” will be used<br />

throughout this directory. You'll need to call and<br />

make sure the provider you choose is accepting new<br />

patients. If you choose a PCP who is not taking new<br />

patients, we may assign you to a PCP who is. You do<br />

not have to keep seeing the same PCP. You can<br />

choose another PCP who is accepting new patients by<br />

calling UnitedHealthcare Customer Service at the<br />

number listed on the back of this directory.<br />

Your PCP plays an important role in your health care.<br />

They can:<br />

• Help you make smart lifestyle choices to<br />

improve and maintain your health<br />

• Per<strong>for</strong>m preventive care and screen <strong>for</strong> health<br />

conditions<br />

• Help you better understand your medical<br />

conditions<br />

• Coordinate the care you get from specialists to<br />

help prevent duplicate and unnecessary tests<br />

• Manage your prescriptions to avoid possible<br />

harmful interactions<br />

Your PCP can handle most common health care<br />

needs. If you need to see a specialist, your PCP can<br />

recommend an appropriate network specialist and<br />

answer questions about the specialist’s treatment plan.<br />

Your PCP will provide follow-up care as needed.<br />

The "network providers" listed in this directory have<br />

agreed to provide health care services to our members.<br />

Check your Evidence of Coverage to make sure your<br />

plan includes additional benefits or that you've signed<br />

up <strong>for</strong> optional supplemental benefits be<strong>for</strong>e making<br />

appointments <strong>for</strong> dental, vision or hearing services. If<br />

you have been going to one network provider, you are<br />

not required to continue going to that same provider.<br />

In some cases, you may get covered services from<br />

non-network providers.<br />

Getting services from out-of-network<br />

providers<br />

Your Evidence of Coverage has in<strong>for</strong>mation about any<br />

out-of-network services your plan covers, as well as<br />

the rules <strong>for</strong> using these services. If you have any<br />

questions about your plan's benefits or coverage <strong>for</strong><br />

out-of-network services, please call UnitedHealthcare<br />

Customer Service. If you use an out-of-network<br />

provider <strong>for</strong> routine care, your costs may be higher.<br />

You can ask our plan to pay our share of<br />

the cost of your covered services<br />

There may be times when you get a bill from a<br />

provider <strong>for</strong> the full cost of medical care you have<br />

received. In most cases, you should send this bill to us<br />

instead of paying it. UnitedHealthcare will look at the<br />

bill and decide whether the services should be<br />

covered. If we decide they should be covered, we will<br />

pay the provider directly. Please see Chapter 5 of your<br />

Evidence of Coverage <strong>for</strong> more in<strong>for</strong>mation.<br />

Emergency and urgently needed care<br />

If you have a medical emergency, get help as quickly<br />

as possible. Call 911 or go to the nearest emergency<br />

room or hospital. You can always get emergency care<br />

from the nearest available provider, whether or not<br />

they are a network provider. Your plan covers<br />

emergency services whether you are in or out of the<br />

service area. Your plan covers ambulance services <strong>for</strong><br />

getting to the emergency room when any other way<br />

could harm your health. You or someone else should<br />

call and tell us about your emergency care, usually<br />

within 48 hours.<br />
