Returning Student Athletes: Visit www.sowl123.net Login using your existing email and password. Update the My Info section. Under Med History click on General and complete a new Medical History. Next you will go the Forms and download and print the NDSCS pre participation form and the HIPAA form. These forms are to be completed and returned to the Athletic Department when you report to campus in the fall. At the time you report you will also have to provide proof of insurance so have your insurance care with you, copies of the front and back will be made and placed on file with the Athletic Department. Your card will be returned to you. It is the athlete’s responsibility to have the insurance cared for any possible medical visits that may arise. It is also the athlete’s responsibility to provide updates with any changes to the submitted information immediately, so current records are maintained if injury/medical emergency may occur. 2
By submitting your personal information/data you have given NDSCS Athletic Trainers, and staff permission to render any medical care deemed reasonable and necessary for the health and well-‐being of the student athlete, this includes all practice/event, off season training and travel.
