2. Insert <strong>the</strong> code “DECD” on <strong>the</strong> line<br />
after <strong>the</strong> word “Code:” on <strong>the</strong> first<br />
page, upper left-hand corner.<br />
3. Complete <strong>the</strong> information requested on<br />
<strong>the</strong> lines in <strong>the</strong> upper left-hand corner<br />
of <strong>the</strong> first page. Please include area<br />
codes.<br />
4. For <strong>the</strong> section of <strong>the</strong> document<br />
starting with “In <strong>the</strong> ____ Judicial<br />
District”, insert <strong>the</strong> word “8 th ” on <strong>the</strong><br />
first blank and <strong>the</strong> words “<strong>Clark</strong><br />
County” on <strong>the</strong> second blank.<br />
5. Insert <strong>the</strong> wife’s and husband’s names<br />
on <strong>the</strong> lines above those words in <strong>the</strong><br />
caption.<br />
6. Leave <strong>the</strong> lines to <strong>the</strong> right of <strong>the</strong><br />
caption blank. The Clerk’s Office will<br />
give you a case number and department<br />
letter when you file <strong>the</strong> Joint<br />
Petition. Once you get <strong>the</strong> case number<br />
from <strong>the</strong> clerk, you will need to put it<br />
on any documents that you give to <strong>the</strong><br />
Court.<br />
7. The Decree of Divorce uses a fill-in<strong>the</strong>-blank<br />
format and will tell you what<br />
information you need to put into <strong>the</strong><br />
blank. Please be sure to initial <strong>the</strong><br />
applicable lines.<br />

NOTE: YOU SHOULD PREPARE THIS DOCUMENT AFTER<br />
THE JUDGE HAS SIGNED YOUR DECREE.<br />
1. The Certificate of Mailing is a<br />
document that shows <strong>the</strong> Court that you<br />
served <strong>the</strong> o<strong>the</strong>r party with a copy of<br />
<strong>the</strong> document you filed.<br />
