women are able to appreciate the importance <strong>of</strong> pre-conceptual modification <strong>of</strong> their anticonvulsant<br />

regimens and will be motivated to seek pre-conceptual care.<br />

<strong>The</strong> recommendations prefac<strong>in</strong>g this section <strong>of</strong> the Guidel<strong>in</strong>e are based on those items <strong>of</strong> <strong>in</strong>formation<br />

which the Consensus Guidel<strong>in</strong>e <strong>of</strong> Delgado-Escueta and Janz 5 suggests should be made available to<br />

all epileptic women <strong>of</strong> child-bear<strong>in</strong>g age. <strong>The</strong> SOGAP Group commends the <strong>in</strong>formation leaflets for<br />

women produced by the Royal College <strong>of</strong> Midwives/Jo<strong>in</strong>t Epilepsy Council <strong>of</strong> the UK and Ireland,<br />

Guidel<strong>in</strong>es for <strong>Women</strong> <strong>with</strong> Epilepsy 10 , and by the Jo<strong>in</strong>t Epilepsy Council, Choices: <strong>Women</strong> and<br />

Epilepsy 11 , and suggests that these be made available to women <strong>in</strong> General Practice and Specialist<br />

Cl<strong>in</strong>ic sett<strong>in</strong>gs from the po<strong>in</strong>t <strong>of</strong> diagnosis <strong>of</strong> epilepsy onwards.<br />

<strong>The</strong> full benefits <strong>of</strong> peri-conceptual care can only be realised if pregnancies <strong>in</strong> women <strong>with</strong> epilepsy<br />

are planned pregnancies. <strong>Women</strong> on the enzyme-<strong>in</strong>duc<strong>in</strong>g anticonvulsants (phenobarbitone,<br />

phenyto<strong>in</strong>, primidone, carbamazep<strong>in</strong>e and possibly also topiramate) have particular problems <strong>with</strong><br />

contraception (and therefore <strong>with</strong> achiev<strong>in</strong>g planned pregnancies) as these drugs <strong>in</strong>crease the rate <strong>of</strong><br />

metabolism <strong>of</strong> contraceptive steroids 12 . <strong>The</strong> <strong>in</strong>tra-uter<strong>in</strong>e device may be a suitable contraceptive<br />

method for many women <strong>with</strong> epilepsy. Those who prefer to use hormonal methods should have<br />

treatment modified <strong>in</strong> l<strong>in</strong>e <strong>with</strong> current recommendations which are summarised below<br />

Hormonal Contraception for <strong>Women</strong> tak<strong>in</strong>g Enzyme-<strong>in</strong>duc<strong>in</strong>g Anticonvulsants (phenobarbitone,<br />

phenyto<strong>in</strong>, primidone, carbamazep<strong>in</strong>e)<br />

This advice comprises Grade C recommendations extracted from the FPA Contraceptive Handbook 12<br />

and Handbook <strong>of</strong> Family Plann<strong>in</strong>g 13<br />

Comb<strong>in</strong>ed oral Contraceptive Pill (COC)<br />

• Use a 50µg oestrogen pill (eg Nor<strong>in</strong>yl-1, Ovran).<br />

• If break through bleed<strong>in</strong>g (BTB) occurs, comb<strong>in</strong>e pills to provide 80 or 100 (maximum) µg<br />

oestrogen.<br />

• <strong>The</strong> use <strong>of</strong> 4 packs <strong>of</strong> COCs consecutively <strong>with</strong> a reduced pill-free <strong>in</strong>terval <strong>of</strong> 4 days after the<br />

fourth pack is recommended. Such a regimen provides enhanced contraceptive cover and can<br />

reduce the frequency <strong>of</strong> seizures if hormonally triggered.<br />

• Ma<strong>in</strong>ta<strong>in</strong> extra contraceptive cover for 8 weeks if enzyme-<strong>in</strong>ducers <strong>with</strong>drawn.<br />

Progestogen-only pill (POP)<br />

• Probably best avoided. If no other method acceptable, doubl<strong>in</strong>g the daily dose <strong>of</strong> POP reported to<br />

be effective.<br />

Depot progestogen (Depo-provera)<br />

• Reduce <strong>in</strong>terval between depo-provera <strong>in</strong>jections from 12 weeks to 10.<br />

Progestogen implants (Norplant)<br />

• Currently, not recommended <strong>in</strong> long-term users <strong>of</strong> enzyme-<strong>in</strong>duc<strong>in</strong>g drugs.<br />

Cl<strong>in</strong>icians should ensure that women <strong>with</strong> epilepsy are aware <strong>of</strong> the support and advice which is<br />

available to them through the Epilepsy Association <strong>of</strong> Scotland (0141 427 4911).<br />
