With regards to <strong>the</strong> CAH disease report<strong>in</strong>g system, such systems have considerable potential to<br />

contribute to national disease surveillance. In Tanzania for example, those pastoral districts with a<br />

CAHW report<strong>in</strong>g system out‐performed all o<strong>the</strong>r districts <strong>in</strong> terms <strong>of</strong> report submissions 24 and this is<br />

one reason why <strong>the</strong> OIE supports well‐tra<strong>in</strong>ed and supervised CAHWs. The approach <strong>in</strong> Mandera is<br />

a useful start but could be streng<strong>the</strong>ned by reference to <strong>the</strong> six ma<strong>in</strong> <strong>in</strong>dicators <strong>of</strong> a surveillance<br />

system viz. sensitivity, specificity, timel<strong>in</strong>ess, representativeness, simplicity and acceptability 20 .<br />

Use <strong>of</strong> veter<strong>in</strong>ary drugs by pastoralists<br />

In general, good CAHW tra<strong>in</strong><strong>in</strong>g and support systems recognize that pastoralist tra<strong>in</strong>ees have strong<br />

diagnostic skills, and are knowledgeable on <strong>the</strong> cl<strong>in</strong>ical signs and basic epidemiology <strong>of</strong> many<br />

livestock diseases. In contrast, <strong>the</strong> ma<strong>in</strong> deficit <strong>in</strong> <strong>the</strong> knowledge <strong>of</strong> CAHW tra<strong>in</strong>ees is <strong>the</strong> correct use<br />

<strong>of</strong> veter<strong>in</strong>ary medic<strong>in</strong>es. Although <strong>the</strong> assessment did not aim to study this issue <strong>in</strong> any detail, <strong>the</strong><br />

field work <strong>in</strong>dicated that some untra<strong>in</strong>ed pastoralists were misus<strong>in</strong>g veter<strong>in</strong>ary drugs.<br />

• Many <strong>of</strong> <strong>the</strong> pastoral <strong>in</strong>formants were treat<strong>in</strong>g <strong>the</strong>ir animals us<strong>in</strong>g 500mg oxytetracycl<strong>in</strong>e capsules,<br />

adm<strong>in</strong>istered by <strong>in</strong>jection; this treatment was used particularly for respiratory problems ma<strong>in</strong>ly <strong>in</strong><br />

goats and camels, and <strong>the</strong> diluted drug was directly <strong>in</strong>jected <strong>in</strong>to <strong>the</strong> thoracic cavity between <strong>the</strong><br />

third and fourth ribs. On average, a s<strong>in</strong>gle dose <strong>of</strong> 12 capsules was used to treat an adult camel<br />

and three capsules to treat an adult goat, <strong>in</strong>dicat<strong>in</strong>g that <strong>the</strong> dose <strong>of</strong> this treatment was around 10<br />

times higher than <strong>the</strong> 20 mg/kg body weight dosage recommended by drug manufacturers for<br />

<strong>the</strong>se species. This practice needs fur<strong>the</strong>r <strong>in</strong>vestigation as its widespread use <strong>in</strong>dicates that herders<br />

perceive it to be beneficial.<br />

• Some pastoralists were adm<strong>in</strong>ister<strong>in</strong>g antibiotic tablets orally to adult rum<strong>in</strong>ants, but also<br />

report<strong>in</strong>g that treated animals <strong>the</strong>n became emaciated. This was perhaps due to damage caused to<br />

<strong>the</strong> normal rumen flora by <strong>the</strong> antibiotic.<br />

• Pastoralists may prefer us<strong>in</strong>g antibiotic tablets ra<strong>the</strong>r than <strong>in</strong>jection because oral adm<strong>in</strong>istration is<br />

easier than <strong>in</strong>ject<strong>in</strong>g, and <strong>the</strong> tablets are relatively cheap.<br />

These experiences <strong>in</strong>dicate that <strong>the</strong> CAH system could be supported by mass media <strong>in</strong>formation<br />

campaigns or <strong>in</strong>formation distributed through <strong>the</strong> pharmacies, to raise awareness about <strong>the</strong> correct<br />

use <strong>of</strong> veter<strong>in</strong>ary drugs.<br />

5. Conclusions and Recommendations<br />

The impact assessment reached <strong>the</strong> follow<strong>in</strong>g conclusions.<br />

• The veter<strong>in</strong>ary drug supply system based on rural, private veter<strong>in</strong>ary pharmacies supply<strong>in</strong>g<br />

CAHWs <strong>of</strong>fered good potential for f<strong>in</strong>ancial susta<strong>in</strong>ability, particularly if a veter<strong>in</strong>ary drug<br />

company(s)/supplier could service <strong>the</strong> pharmacies through normal bus<strong>in</strong>ess contracts and credit<br />

arrangements. The establishment <strong>of</strong> an effective CAH system without directly provid<strong>in</strong>g material<br />

or cash <strong>in</strong>puts <strong>in</strong>to <strong>in</strong>dividual bus<strong>in</strong>esses (<strong>in</strong>clud<strong>in</strong>g CAHWs kits) is an appropriate approach both<br />

for <strong>the</strong> VSF Suisse/ELMT project and probably, with<strong>in</strong> pastoral areas <strong>of</strong> Kenya more widely.<br />

• The role <strong>of</strong> VSF Suisse/ELMT as a provider <strong>of</strong> collateral to local veter<strong>in</strong>arians and CAHWs is one<br />

<strong>of</strong> <strong>the</strong> key lessons learned from this assessment. It enabled veter<strong>in</strong>ary workers who may not have<br />

o<strong>the</strong>rwise afforded <strong>the</strong> <strong>in</strong>itial <strong>in</strong>vestment required, to streng<strong>the</strong>n private veter<strong>in</strong>ary bus<strong>in</strong>ess. This<br />

approach should be replicated by o<strong>the</strong>r NGOs with a focus on promotion <strong>of</strong> veter<strong>in</strong>ary service<br />

privatization <strong>in</strong> pastoral areas. The authors understand that <strong>the</strong> mutual benefits‐based<br />

collaboration between <strong>the</strong> local private drug suppliers and CAHWs is essential to ensure <strong>the</strong><br />

susta<strong>in</strong>ability <strong>of</strong> a CAH system.<br />
