<strong>Referral</strong> Parent Follow Up Report<br />

If a community agency chooses to complete their own referral parent follow up<br />

surveys, then that agency needs to submit a Parent Follow Up Report quarterly to<br />

CDD. The report needs to be submitted electronically to the Consumer Education and<br />

<strong>Referral</strong> Consultant in the month following the end of the quarter. The report will need<br />

to represent 20% of the parents who received services for that quarter. The reporting<br />

form and survey tool are as follows:<br />
