How To Use This Directory
IMPORTANT NOTE: The providers listed in this directory are available to participants in the
WEA Trust Preferred Provider Organization (PPO) East State Health Plan. The information in this directory
was current at the time of printing. However, because providers can change their address or new providers
can join the network over time, please go to WeaTrustStateHealthPlan.com to see the most up-to-date
information on participating providers.
You can also contact our Customer Service Department at (800) 279-4000, or you can visit our Web site at
WeaTrustStateHealthPlan.com and click on the Find a Doctor feature to learn more about the participating
providers in the WEA Trust PPO East Network. If you decide to receive care from a nonparticipating provider,
we strongly recommend that you contact our Customer Service Department to learn more about any
financial implications of receiving care outside the Network.
If your provider is not a participant of the WEA Trust PPO East State Health Plan Network and you’d like for
him or her to join, ask your provider to call us at (800) 279-4000 for more information about joining the
Network.

• Except for urgent or emergency situations, care received outside the PPO service area will be paid
as non-network—subject to coinsurance and deductibles—if it is not authorized in advance by
WEA Trust. If a physician you are interested in seeing is not listed in this directory, call
(800) 279-4000 to find out about specific provider participation.
