Treatment algorithm <strong>for</strong> buprenorphine maintainence <strong>for</strong><br />

opioid dependent patients<br />

I. DETERMINATION OF DAILY DOSE REQUIREMENT<br />

Patient Selection<br />

Above 21 years <strong>of</strong> age<br />

Regular opioid use <strong>for</strong> at least 5 years<br />

At least 2 unsuccessful abstinent attempts from recognized treatment centres<br />

Not suitable <strong>for</strong><br />

maintenance treatment<br />

�Pregnancy<br />

�Polydrug use<br />

�Concomitant psychiatric condition<br />

To assess later on<br />

<strong>for</strong> maintenance treatment<br />

Initiation <strong>of</strong> induction<br />

(BPN) 2mg or 4mg s.l. to be given<br />

as soon as OWS* starts<br />

Observe (OWS) withdrawals <strong>for</strong> 2 hours<br />

OWS (+) OWS (-)<br />

Give 2mg BPN s.l<br />

Buprenorphine 2 mg can be added next day if required (total 8mg/day). Subsequent increase in doses<br />

should preferably be more slow and every 2-3 days at the rate <strong>of</strong> increment <strong>of</strong> 2mg/day only.<br />

Maximum dose that has been used in our clinical setting is 24 mg/day but such dose requirements are<br />

rare and usual daily dose is in range <strong>of</strong> 4-8 mg/day and some patients may require upto 12 mg/day.<br />

*Opioid withdrawal syndrome
