Who can submit the <strong>NOI</strong> <strong>and</strong><br />
<strong>BAMM</strong> request?<br />
• Each <strong>NOI</strong> <strong>and</strong> <strong>BAMM</strong> request must be submitted by<br />
the Designated Representative, or an alternate<br />
designated representative, or agent.<br />
• A single Designated Representative may represent<br />
multiple facilities.<br />
• An association cannot submit <strong>NOI</strong> or <strong>BAMM</strong><br />
requests for multiple facilities.<br />
