3Hospice-based CommunityPalliative Care TeamThe team is employed by Wigan and LeighHospice and is equivalent to Macmillan teamselsewhere. They offer specialist support andwork in close liaison with the patient’sGeneral Practitioner and Primary Care Teamin:● supporting the patient and their carer(s)at home● informing the patient and their carer(s)about issues relating to the patient’sillness● advising on symptom control.Before visiting, the team requires a referralfrom the patient’s General Practitioner,District Nurse or Hospital Consultant and, inevery case, the approval of the GeneralPractitioner.The team do not offer ‘hands-on’ nursingcare.Physiotherapy and OccupationalTherapy ServicesThe role of the rehabilitation team is tosupport the patient and carers as part of theHospice inter-disciplinary team. The primaryaim is to help the patient achieve theirmaximum potential function within their ownlimits and within a realistic time scale,adjusting goals and treatments according totheir abilities and stage of illness.
