13. A properly executed non-collusion statement must accompany all bids.14. Due to the uncertainty of the cost of asphalt cement used in the manufacture of <strong>bituminous</strong><strong>concrete</strong>, a price adjustment clause is included. Price adjustment shall be based on NewYork State Department of Transportation methods and the price adjustment shall be based onthe September 2012 F.O.B. average terminal price of $576.000 per English Ton.15. The <strong>bituminous</strong> paver shall be a Barber-Greene SB-260 or equal equipped with automatictransverse slope and longitudinal grade screed controls. The <strong>bituminous</strong> paver shall befurnished with two (2) competent operators.16. The paver shall be equipped with a screed that is fully extendible from 10 feet to 19 feet 6inches. Heat and vibration shall be provided for the full, extended width. The extendersshall have independent slope control and on the go capabilities to change width and slope.17. The compaction roller shall be a Ingersol – Rand Model DD-110 compaction roller, or equal,which will be used in compacting the pavement mat to at least 95% of the design density.The roller shall be furnished with a competent operator.PREVAILING WAGES AND SUPPLEMENTSThe Contractor shall conform to Section 220 of the New York State Prevailing Wage Rates for<strong>Cortland</strong> <strong>County</strong> as provided by the New York State Department of Labor, as contained in thisproposal. The Prevailing Wage Rate document to be used is available for viewing at thefollowing web link: http://www.cortland-co.org/Highway/index.htm, under link entitled“Material and Service Bids”.The Contractor shall maintain appropriate payroll records on each employee and file a certifiedpayroll weekly. Each payroll record must be affirmed as true under the penalties of perjury,which means a notarized signature to that effect. If during the period of the contract theContractor is cited by the NYS Department of Labor for a willful violation of the laws andregulations applicable to the bidding process, pricing policies, or in any way applicable to theservices or supplies rendered pursuant to this contract, the <strong>County</strong> shall have the option to deemthe Contractor irresponsible and in breach of contract. The <strong>County</strong> may then award the work to adifferent contractor.CONTRACT AND INSURANCEIf awarded the contract and prior to beginning any work, the Contractor shall be required toexecute the <strong>County</strong> of <strong>Cortland</strong>’s Standard Contract Agreement and provide the <strong>County</strong> of<strong>Cortland</strong> proof of insurance coverage as listed in the Standard Contract Agreement. Notice ishereby given that <strong>Cortland</strong> <strong>County</strong> requires said insurance coverage by way of a Certificate ofInsurance naming the <strong>County</strong> of <strong>Cortland</strong> as an additional insured party.
