The explanation is that the average life-span of a femaleanopheline is only a little longer than the extrinsic incubation period. Thus, a20% reduction of the average life-span of female anophelines may lead to asituation, where no or very little transmission takes place. In contrast, even if thedensity of anophelines is reduced by, for example 80%, the remaining 20% willbe able to maintain some transmission. In practice, methods which reduce thelife-span of anophelines may also reduce their density and the frequency withwhich they bite humans.1.8.3 Behaviour Change Communication (BCC)The NVBDCP envisages strong community participation and behavior changecomponents in the malaria control program to meet the challenges in malariacontrol. Three interventions of proven value are now being introduced at a largescale into the program, each of which has benefits tangible even to the layperson, and thus having high likelihood of acceptability and utilization:1.8.3.1 Diagnosis. In the place of slide tests which involved delay in gettingresults, rapid diagnostic tests (RDT) for P. falciparum are now available. Thesetests can be conducted at the most peripheral levels by any one with simpletraining.1.8.3.2 Treatment. In place of Chloroquine which was associated with treatmentfailure due to drug resistance, ACT is now available which is nearly 100%effective and is not associated with any major side effects.1.8.3.3 Bed nets. In place of Insecticide impregnated Bednets which requiredperiodic reimpregnation, soon we will have Long lasting bed nets which do notrequire reimpregnation, remaining effective even after 20-25 washes and lastingfor 3-5 years.8
A fourth component of the program having high acceptance potential is theestablishment of trained ASHAs at village level, known as ASHAs. The malariacontrol program offers considerable scope for communities to participate in andown the program.1.9 Vulnerable groupsCertain groups are particularly vulnerable to malaria:Pregnancy increases vulnerability of women to severe malaria, by loweringimmunity. Malaria can cause abortion, stillbirth, low birth weight and severeanaemia in pregnant woman. Early and complete treatment of malaria istherefore of the greatest importance in pregnant women.Young children are at highest risk in those populations, which are exposed tovery intense transmission, where older people develop immunity. Even in areaswith less intense transmission, severe disease may develop particularly rapidly inyoung children. Diagnosis may be difficult, as young children can have feverfrom a number of different causes.People who do not live in malaria-endemic areas have no immunity. The problemof immigrants may be a lack of knowledge of malaria and about where to go fortreatment if they fall ill. Travellers, tourists and immigrants need information onprotective measures against malaria in various locations and situations.1.10 Malaria and genderWomen are more likely to delay visits to qualified health care providers and morelikely to visit traditional healers for their sickness and for their children because oftheir lesser control over resources and decision-making process in thehousehold. There may be a gender bias towards the male child, who getspreferential attention in getting health care. Studies have shown that women inmany situations do not have control over decision making about accessingquality health care. Malaria in pregnant women is associated with more seriouscomplications.When men have malaria the household becomes severely affected economically.Women’s work days become longer and the work load becomes heavier as theyhave to take care of the ill apart from their routine activities. There may also betimes when they have to go for work to compensate for the wage loss of the malemembers. In households without any male earning member, the economicconsequence can be very severe on the entire family.There is a need to promote women's active participation in leadership anddecision-making. It is critical that people at every level come together to createawareness of the magnitude of the problem and on the way gender inequalitieslead to a greater impact on women and the girl child in case of sickness due tomalaria.9
