Instructions for Completing Title 71 Amendment FormPLEASE TYPE THIS FORM: At least a 10-point typed fontmust be used when completing the form.RETURN NAME/ADDRESS: Type the name and addresswhere you want the Secretary of State’s office to return theacknowledgement letter.VERIFY INFORMATION: Verify all information you enter onthe form for accuracy and correct spelling. An error mayresult in your lien becoming ineffective.MONTANA ORIGINAL FILING NUMBER: You must list theoriginal filing number. Be sure to check the number foraccuracy. Please do not reflect the filing number for anamendment on this form. Only one filing number per form isallowed.ORIGINAL FILING DATE: Reflect the date the original lienwas recorded with our office.ORIGINAL DEBTOR NAME: (1a or 1b) List a debtor namethat appears on our records. This is to ensure we areamending the correct lien. We require only one originaldebtor name on the form. Address information is notrequired.ORIGINAL SECURED PARTY NAME: (2a or 2b) List asecured party name that appears on our records. This is toensure we are amending the correct lien. We require onlyone original secured party name on the form. Addressinformation is not required.UPDATED DEBTOR NAME (3a or 3b): These fields areused when adding, changing or deleting a debtor’s name,address, social security or tax ID number. You can list eitheran individual name or an organization name for the debtor.But do not list both entity types in the box.UPDATED COLLATERAL: These fields are used whenadding, changing or deleting crops. Note: You must list thespecific crop such as wheat, barley, hay. The listing all of “allcrops” will not suffice. In addition, the legal land descriptionwhere the crops are located is required in addition to thecounty.CROP YEAR: For a crop grown in soil, the calendar year inwhich it is harvested or to be harvested.COUNTY: Where the crop is produced and/or located.QUANTITY/DESCRIPTION: Number of bushels and legalland description where crop is produced/located. Specify theunits. Example: 300 bushels of winter wheat.ACKNOWLEDGMENT LETTER: A system derivedacknowledgement letter will be returned to you to retain withyour records.
