Numerator: Residents who did not have their medications reviewed by a doctor or pharmacist in the three months prior to the most recent assessment. Denominator: All residents on most recent assessment. Personal Care 9. Poor Hydration Status Definition: Prevalence of dehydrated residents. Numerator: Number of residents with two (2) or more indicators of poor hydration status on most recent assessment. Denominator: All residents on most recent assessment. Resident Lifestyle 14. Meaningful Activity Definition: Prevalence of little or no participation in meaningful activity. Numerator: Residents with little or no meaningful activity (score of 2 or 3) at most recent assessment. Denominator: All residents on most recent assessment.
