Beginning in October 2006, the dental licensure renewal period extended from two years to three. This change affects both dentists and<br />

dental hygienists. Continuing education (CE) requirements have been increased by one-third, and dentists will be required to show proof of<br />

a minimum of 48 hours of continuing education over the three-year licensing period. Hygienists will be required to show proof of 36 hours<br />

of continuing education over the three-year period.<br />

As an <strong>Illinois</strong> licensed dental professional, it is your responsibility to keep evidence of your CE activities for five (5) years following the<br />

license renewal in the event you are audited by the <strong>Illinois</strong> Department of Financial and Professional Regulation, Division of Professional<br />

Regulation. You should maintain your CE records in the same manner in which you keep Internal Revenue Service records.<br />

CE sponsors are responsible for providing a proof of attendance and shall maintain attendance records for at least five<br />

years. The proof of attendance shall contain the following:<br />

• The name and address of the sponsor;<br />

• The name, address and license number of the participant;<br />

• A brief statement of the subject matter;<br />

• The number of hours attended;<br />

• An indication of whether the program fulfills CE requirements for dentist, dental hygienist or both;<br />

• The date and place of the program; and<br />

• The signature of the sponsor.<br />

CE credit may be earned for completion of an individual study course (correspondence, audio or video course) sponsored by an approved<br />

<strong>Illinois</strong> sponsor. Such courses shall include a test that the licensee must pass to obtain credit. No more than 50% of the required CE credit<br />

hours during a prerenewal period may be acquired through correspondence/self study courses.<br />

Also note, courses not acceptable for CE include, but are not limited to, estate planning, financial planning, investments and personal health.<br />

The <strong>Illinois</strong> <strong>State</strong> <strong>Dental</strong> <strong>Society</strong> no longer maintains members’ individual CE records. For your convenience, we have<br />

prepared a form to log your CE credits on page 7. Store this sheet and your CE records in a file folder in the event of an audit. Feel<br />

free to make additional copies of the log sheet.<br />

ISDS reserves the right to cancel a course. In the case of course cancellation, your fee will be fully refunded or transferred to the next<br />

course date. Individual cancellations received at least 15 days prior to the course will receive a full refund. After this period, only an<br />

85% refund will be given. Cancellations must be made prior to the day of the course to receive the 85% refund. No refunds will be<br />

granted for no-shows. A course credit will be given due to illness and weather conditions.
