Our ExecutiveSummaryWhen things go wrong in our local hospitals ithits the headlines,and the quality, safety andfinancial issues experienced by our hospital Trusthave been well publicised in recent years. Butthe less well known story in Morecambe Bay isnot about what happens inside hospitals – it’sabout what isn’t happening in the primary,community and social care services that most ofus rely on for most of our healthcare needs.Precious lives are being lost because we aren’tdoing enough to support people to prevent themfrom becoming ill in the first place. Then, whenpeople are ill, we don’t always give them theadvice or tools to manage their own conditionand prevent crises from occurring - so all toooften they end up in a hospital bed with all theupheaval and distress that this can involve.Our local communities deserve much more. Weowe it to them to be clear about what has gonewrong and why and our proposals for fixing thisfor the future, so that we can restore publicconfidence in local health and care services.This is why all the health and social careorganisations that deliver services across the Bayhave united under the banner of the “Better CareTogether” programme to co-design high qualityadult, children’s and mental health services thatwill be safe, affordable and fit for the future.The result is a range of proposals shaped aroundour unique geography and demographics, and isbased on sound clinical evidence, national andinternational best practice and detailed analysisby independent third parties. This work has beenled by hospital doctors, GPs, nurses and socialcare colleagues who have been passionateabout finding out what local people and ourpartners think of their health and care servicesand their aspirations for the future. The findingsof this extensive engagement have influencedthe solutions that we are proposing.At the heart of our Strategy is a “population”based approach to promoting wellbeing andproviding care in which people and their needsare the focus rather than processes andbuildings. Responsibility for health and care willbecome a true partnership between the peopleneeding to access services and those whoprovide them. People will be enabled to makelifestyle choices that will keep them healthy forlonger and to take control and manage theirlong term conditions with local clinical teamsintegrating the support and technology theyneed around them. This will include moreeffective use of video links, texting and othertelehealth facilities which will reduce the needfor patients to travel whether from their own ortheir nursing care home.Our local GP practices will become the way forpeople to gain access to all care includinghospital services. GPs will work closely with awide range of other health and social carecolleagues to ensure people’s needs are met,and to cut through the red tape that causesunnecessary barriers, delays and expense in theexisting system. Hospital consultants will workalongside these new teams in the community toshare their expertise, so that more staff are ableto help patients cope with the long termconditions that so many of us will encounter inour later years.5
