MOON BOUNCE/JUMPER REQUIREMENT AND INSURANCEAn outdoor shelter reservation is required in order to have a jumper. No indoor room reservations will be allowedto have a jumper.An additional non-refundable $50 fee will be added to the reservation fee.Only standard size jumpers are allowed. No water or slide features are permitted.APPLICANT must provide generator for moon bounce.Pro<strong>of</strong> <strong>of</strong> Insurance is required. APPLICANT must obtain a Certificate <strong>of</strong> Insurance from the jumper <strong>rental</strong> companyat their expense. The <strong>City</strong> requires the Certificate <strong>of</strong> Insurance to be in the Accord format, sample available uponrequest.APPLICANT must pay the jumper fee and provide a copy <strong>of</strong> the Certificate <strong>of</strong> Insurance no later than two weeksprior to the reservation date. Certificates must be brought in person to the Parks and Recreation <strong>of</strong>fice. No faxesor emails are accepted. No exceptions.APPLICANT or GROUP whose name appears on the reservation must secure insurance in the amount <strong>of</strong> onemillion dollars ($1,000,000.00) which holds the <strong>City</strong> <strong>of</strong> <strong>Downey</strong> harmless for any damage or injury. The <strong>City</strong> <strong>of</strong><strong>Downey</strong> must be named as “additional insured.”APPLICANT shall indemnify the <strong>City</strong> <strong>of</strong> <strong>Downey</strong> and its <strong>of</strong>ficers, <strong>of</strong>ficials, agents, and employees against and willhold the same harmless from any and all actions, claims, damages to persons or property, penalties, obligations,or liabilities which may be asserted or claimed by any person, firm, entity, corporation or other organization arisingout <strong>of</strong> or in connection with the use <strong>of</strong> <strong>City</strong> facilities by said applicant.STAFF’S RESPONSIBILITIESStaff is responsible for facilitating the reservation and will only deal with the applicant listed on the reservationform.Staff is required to conduct reservation checks <strong>of</strong> the event at minimum every 60 minutes or more as needed.Staff is responsible to check and change trash receptacles.Staff is responsible to monitor activity to ensure compliance.APPLICANT’S RESPONSIBILITIESIt is the responsibility <strong>of</strong> the APPLICANT to abide by all <strong>facility</strong> reservation <strong>policies</strong> and <strong>park</strong> rules.Failure to comply with any single item, portion <strong>of</strong>, or combination <strong>of</strong> <strong>policies</strong> and <strong>procedures</strong> or misrepresentation<strong>of</strong> the reservation will result in the <strong>City</strong>’s right to refuse future service to said applicant.For more information, please call the Parks and Recreation Department at (562) 904-7238.Page 6 <strong>of</strong> 66/19/13 SM
