<strong>Prescription</strong> <strong>Drug</strong> <strong>Monitoring</strong> <strong>Programs</strong>: An Assessment <strong>of</strong> the Evidence for Best Practices 42comparatively well-‐established <strong>PDMP</strong>s, awareness <strong>of</strong> and enrollment in a <strong>PDMP</strong> do not always entail itsutilization (Feldman et al., 2011).1. Conduct promotional campaignsRationale and evidence for effectiveness: As noted above, states have undertaken outreach initiatives toinform practitioners and the public about the benefits <strong>of</strong> consulting the <strong>PDMP</strong> to help assure safeprescribing and dispensing. They have also sought to provide reassurances about patient privacy andexplain that fears about the so-‐called chilling effect (unwarranted reductions in prescribing painmedication as a consequence <strong>of</strong> prescription monitoring) may be overblown. Published data are scarceon the impact <strong>of</strong> consulting a <strong>PDMP</strong> on prescribing, but a recent study <strong>of</strong> emergency physicians indicatedthat when informed <strong>of</strong> a patient’s prescription history, they prescribed more controlled substances forsome patients but less for others, when compared to their not being informed <strong>of</strong> patient histories(Baehren et al., 2010). A Canadian study found no significant differences in changes in opioid dispensingrates between provinces with and without <strong>PDMP</strong>s (Fischer et al., 2011). Dissemination <strong>of</strong> these and anysimilar findings that eventually come to light might encourage <strong>PDMP</strong> utilization byallaying prescribers’ concerns about intrusive monitoring <strong>of</strong> their medical practice and any chilling effectthis might have (Barrett & Watson, 2005; Twillman, 2006; Fornili & Simoni-‐Wastila, 2011). Further studyis needed to understand how utilization <strong>of</strong> <strong>PDMP</strong> data influences prescribing decisions.The impact <strong>of</strong> promotional campaigns on utilization will be reflected in the number <strong>of</strong> data queries to the<strong>PDMP</strong>, comparing the periods before and after the campaigns, although controlling for confoundingfactors may prove difficult. Recent data analyses from Virginia suggest that a well-‐focused outreachcampaign, along with program improvements, can increase both enrollment and utilization by prescribersand dispensers (Virginia <strong>Prescription</strong> <strong>Monitoring</strong> Program, 2010). It is likely that other states couldproduce similar analyses to help evaluate the effectiveness <strong>of</strong> their campaigns.Current adoption status: As noted above (see 2. Conduct recruitment campaigns), states have mounteda variety <strong>of</strong> promotional efforts to recruit <strong>PDMP</strong> users and educate them concerning the use and value <strong>of</strong><strong>PDMP</strong> data. Massachusetts has mandated prescriber education for the prescribing <strong>of</strong> controlledsubstances; such education includes information on how to download and interpret prescription historydata (communication with Massachusetts <strong>PDMP</strong>).
