Therefore,whenever mixing parts, the final assembly must always be tested before being put into service. Failure to test couldresult in a sudden failure of the coupling causing severe personal injury or death.• Couplings do wear out. In order to avoid severe bodilyinjury or death, couplings must be routinely checked forcondition and replaced if necessary.• Over-pressurization could result in a suddenfailure of the coupling causing severe bodily injuryor death.
