The Specimen Application Form given here is only for the private candidates.Every private candidate should read and understand this notice completely and complete the Application Form.A special consideration should be given to write the accurate number when mentioning the town number andsubject numbers in the Application.02. Application Forms for school candidatesThe relevant application forms have been sent in order to receive by 01.06.2011 for the governmentschools and approved pirivenas in which the students have sat the G.C.E. (O/L) Examinations before.If any school has not received the applications by now such schools should inform theSchool Examinations Organization Branch immediately. Only those who have leftschool can apply as private candidates. Any one applying as a private candidatewhile still being in school will be debarred from sitting examinations conducted bythe Department of Examinations.03. Private CandidatesAny person not attending any state school, non fee-levying approved school or recognized Pirivenamay sit as a private candidate. Applications for this examination will be received from privatecandidates only up to 30 th June 2011.04. Private candidates should select for the examination the town closest to their permanentresidence.05. Application Forms should be prepared by candidates themselves in accordance with the SpecimenApplication Form appearing in this notice.06. The Application Form should be 28cm x 21cm in size and cages 01-12 should be on the first pagewhile the cages 13-14 should be on the reverse.07. Completed Applications should be sent by registered post to the "Commissioner General ofExaminations, School Examinations Organization Branch, P.O. Box 1503, Colombo". On the topleft-hand corner of the envelope, please write legibly.“GCE (O/L) - December 2011 ” and also the “ Town No.………………………..”08. One Candidate should submit only one Application. If any candidate sends more than oneapplication his candidature will be cancelled / results even though released, will be annulled.They also can be subjected to a long term suspension from the examination. Furthermore oneenvelope should contain only one application. Applications should be completed correctly and clearlyin one's own handwriting and for that matter given instructions should be followed carefully.09. In case you use application forms prepared by outsiders make sure that you enter the Subject Numbersand Town Numbers correctly as they appear below this notice. Before you post the application readit once again and check against this notice to make sure that the numbers and other detailsentered are correct.10. Proof of Identity at the Examination:Candidates should prove their identity at the examination centre each time they sit a paper. Examination FeesAll candidates other than those of the state schools, non fee-levying private schools or approvedpirivenas must pay the prescribed fees.The prescribed fee has to be paid to a post office / a sub post office and the receipt obtained has to bepasted in the relevant cage in the application and forwarded.
