ADVANCE DIRECTIVES<strong>Peach</strong> <strong>State</strong> is committed to ensuring that its members know of, and are able to avail themselvesof, their rights to execute advance directives. <strong>Peach</strong> <strong>State</strong> is equally committed to ensuring thatits providers and staff are aware of and comply with their responsibilities under federal and statelaw regarding advance directives.PCPs and physicians delivering care to <strong>Peach</strong> <strong>State</strong> members must ensure adult members 18years of age and older receive information on advance directives and are informed of their right toexecute advance directives. <strong>Provider</strong>s must document such information in the permanentmedical record.<strong>Peach</strong> <strong>State</strong> recommends to its PCPs and physicians that:• The first point of contact in the PCP’s office should ask if the member has executed anadvance directive; the member’s response should be documented in the medical record• If the member has executed an advance directive, the first point of contact should ask themember to bring a copy of the advance directive to the PCP’s office and document thisrequest• An advance directive should be included as a part of the member’s medical record,including mental health directives• If an advance directive exists, the physician should discuss potential medicalemergencies with the member and/or family member/significant other (if named in theadvance directive and if available) and with the referring physician, if applicable.Discussion should be documented in the medical record• If an advance directive has not been executed, the first point of contact within the officeshould ask the member if they desire more information about advance directives• If the member requests further information, member advance directiveeducation/information should be provided• Member Services and CONNECTIONS representatives will assist members withquestions regarding advance directives; However, no employee of <strong>Peach</strong> <strong>State</strong> mayserve as witness to an advance directive, or as a member’s designated agent orrepresentative<strong>Peach</strong> <strong>State</strong>’s Quality Improvement Department will monitor compliance with this provision duringinitial office site visits and as scheduled thereafter.If you have any questions, regarding advance directives, contact:Quality Department1-800-704-1463www.pshp.com<strong>Provider</strong> Services1-866-874-063302/22/201123
