Once again this should be part of the provider’sSMS. The relevant SOP’s should be part of the pilot’s kit.5.24 Paragraphs 5.16 to 5.23 deal with what this <strong>review</strong> perceives as deficiencies in theSMS. But every risk and “hole” in defences is not always readily discernible,particularly systemic risk in the organisation. Accordingly, SMS’s require continualappraisal and <strong>review</strong> and this is also required by the GBRPSMC 23 . It is not clear how<strong>pilotage</strong> providers fulfil this responsibility.
