By completingthis Registration Form you are providing personal information to the Competition Adviser for theprimary purposes of the Competition Adviser verifying your identity, confirming the valid execution ofthe Registration Form and communicating with you in relation to the Competition. The personalinformation contained in this Registration Form and any personal information obtained by the State,the Competition Adviser or Probity Adviser (and if required the Probity Auditor) in accordance with theCompetition Conditions may be shared between, and used by the State, the Competition Adviser, theProbity Advisor, Probity Auditor, MPV and the Commonwealth of Australia and its other States andTerritories for the above primary purposes, and purposes related to the Competition. MPV may shareyour personal information with its officers, employees, agents and advisers in limited circumstancesrelated to the primary purposes and purposes related to the Competition.If you do not provide the information requested by this form, then the Competition Adviser may not beable to process your Registration Form.
