This request forevaluation must be made at least 2 months in advance of your application to the Nursing Program.Courses from another country cannot be considered for the Nursing Program without an officialtranscript evaluation. You must pay all costs for international transcript evaluations by the NationalAssociation of Credential Evaluation Services (NACRS) and have an official transcript evaluation sent tothe International Admissions Office which is located at the TCC May Owen Center, 1500 Houston Street,Fort Worth, TX 76102. A degree audit can be completed by the applicant in WebAdvisor or with a TCC Advisor or Counselor. Official transcripts from all colleges must be evaluated priorto application. Files are not complete if all transcripts from other colleges or universities have not beensubmitted. If you do not have a transcript evaluation from TCC at the time of your application, you mustsubmit official transcripts and course descriptions of the four required science courses taken at otherinstitutions. It is highlyrecommended that you view the presentation on our website before completing the packet.5. Go to the Trinity River Campus Testing Center to sign up for the HESI A2 Test. Information regardingthe HESI A2 test is addressed in this packet. The 4 hour HESI A2 Test includes math, readingcomprehension, vocabulary & general knowledge, and grammar. The Learning Styles and PersonalityStyle sections will also be given, but not included in scoring. The cost is $37. Availability is limited. Signup early.6. Complete Declaratory Order Process (if required). If you answer “no” to the questions on theenclosed Nursing Eligibility Information Form please sign, date, and return the information form withyour application. If you answer “yes” to any of the questions, it will be necessary for you to be clearedby the Texas Board of Nursing prior to applying to the TCC Nursing Program. This review process cantake a minimum of three months to two years depending on the case. This petition is a formal disclosureto the Board of any eligibility issues that may disqualify you from taking the NCLEX examination forlicensure. Go to www.bon.state.tx.us/ – click on Download Other Applications and Forms and scrolldown to Candidates for Licensure. Click on the Declaratory Order Form and Fingerprint Card. You arerequired to submit the letter from the Board which states you have successfully completed theDeclaratory Order process with your application packet.7. Statement of Student Responsibility.
