Thescoring utilizes data reported bythe <strong>Center</strong>s for Medicare andMedicaid Services.Diabetes Education is Re-CertifiedFCHC’s Diabetes Education Department has expandedits outpatient program to a separate office within the newFCHC Swanton Sleep Lab location.FCHC Diabetes Educationhas renewed its prestigiousEducation RecognitionCertificate from the AmericanDiabetes Association. Thecertification recognizes FCHCas having a quality diabetesself-management educationprogram, which is an essentialcomponent of effective diabetestreatment. The program hasbeen continuously recognizedsince July of 2000.
