Aformal re-evaluation may be done earlier if a patient has had a change in status or an interventionsuch as an epidural steroid injection. The expected range <strong>of</strong> number <strong>of</strong> visits per episode <strong>of</strong> careis 8-24.Discharge PlanningCriteria for discharge1. Independent pain management.2.
